Parse every History-Info row as one ordered cache.

RFC 3261 §7.3 makes repeated list-header rows equivalent to one comma-joined row. The history indices must therefore be validated across the joined value: decoding a later row by itself would incorrectly reject its first 1.1 entry for not beginning at 1.
